Many people delay booking counselling because they assume they need to see their doctor first. Here is the good news: in most cases, you do not. This short guide explains when referrals matter in BC, when they do not, and the one situation where checking first can save you money.
The Short Answer

No. You can book directly with a Registered Clinical Counsellor (RCC) in BC without a doctor’s referral, in person or online. Some extended health plans require a physician’s note before they will reimburse counselling, so check your plan first. Referrals are needed for psychiatrists and some publicly funded programs.
Referral Rules by Provider Type
| Provider | Referral needed to book? | Referral needed for coverage? |
|---|---|---|
| Registered Clinical Counsellor (RCC) | No, book directly | Usually no, but some extended health plans require a doctor’s note for reimbursement |
| Psychologist | No, book directly | Same as above, plan dependent |
| Psychiatrist | Yes, physician referral required | Covered by MSP with referral |
| Publicly funded programs | Often yes, or program intake | Program dependent |
When a Doctor’s Note Matters for Counselling

The referral question for counselling is really an insurance question. A minority of extended health plans will only reimburse counselling if a physician has recommended it. If you plan to use benefits, take two minutes to check your plan booklet or call your insurer before your first session, and ask exactly this: “Do I need a doctor’s referral for counselling with a Registered Clinical Counsellor to be reimbursed?” Our guide to RCC insurance coverage in BC walks through the full coverage check.
For reference, sessions at Sojourn are $160 to $175 per 50 minutes with a Registered Clinical Counsellor, $120 with a Registered Nurse or counselling associate, and $0 to $60 with a master’s level intern, whose first session is free. A referral does not change these fees.
Situations Where You May Want to Involve Your Doctor Anyway
- Medication questions. Counsellors do not prescribe. If medication might be part of your care, your family doctor or a psychiatrist is the right person to talk to, and counselling can run alongside. See our guide to counsellors, psychologists, and psychiatrists in BC.
- ICBC claims. After a motor vehicle accident, ICBC counselling works within your claim; booking soon after the accident matters.
- Complex or unclear symptoms. When physical health could be part of the picture, a medical check alongside counselling is wise.
How Booking Works Without a Referral

- Review counsellor profiles, approaches, and availability online.
- Choose someone whose experience fits your concern and schedule.
- Book directly through the clinic’s booking system.
- Complete intake forms before your first session, no paperwork from your doctor needed.
Frequently Asked Questions
Can I see a counsellor without going to my doctor first?
Yes. Counselling with an RCC in BC is direct access. You can book with a Registered Clinical Counsellor today without any referral.
Does MSP cover counselling if my doctor refers me?
No. A referral does not make counselling MSP covered. MSP covers psychiatrists with a referral, but counselling with an RCC is paid privately or through extended health benefits either way.
My insurer asked for a doctor’s note. What should I get?
Ask your insurer exactly what they need. Usually a short note from your family doctor recommending counselling is enough. Get it before your first session so your receipts qualify from day one.
Do children and teens need a referral for counselling?
No referral is required for child and youth counselling in BC, though consent requirements involving parents or guardians vary with the child’s age and situation. The same insurance note applies if you plan to use benefits.
How soon can I get an appointment without a referral?
Availability varies by therapist rather than by referral status. Our online booking system shows real openings, so you can see the earliest available times before you commit. Evening and weekend clinicians and our master’s level interns often have the soonest availability. If you need to be seen quickly, contact us and we will tell you honestly who has space.
